Dr. Shereen Mahdavi, DPT
Shereen has been a licensed physical therapist for 3 years. She graduated in 2009 from Texas A&M University in College Station with a degree in Kinesiology. She then went on to receive her Master’s in Human Movement from A. T. Still University in 2012. Shereen continued to further her education by graduating from the University of St. Augustine to become a Doctor of Physical Therapy.
Outside of work, Shereen enjoys spending time with family, friends and dog, Dora. Shereen enjoys all Houston sports and the Texas Aggies. In her free time, she participates in spin and Pilates classes and volunteers with the Big Brother Big Sisters program of Houston.
Dr. Garrett Meier
Dr. Garrett Meier, DPT
Garrett joins the Revolve Physical Therapy team having graduated as a physical therapist in 2021. He graduated in 2011 from the University of Arkansas with a degree in Biology. He then went on to receive his Masters in Fitness and Human Performance from the University of Houston – Clear Lake in 2014. He worked at TIRR – Memorial Hermann for several years as a Fitness Specialist for the Strength Unlimited team while he completed his Doctorate in Physical Therapy at the University of St. Augustine.
In his free time, Garrett enjoys playing golf, hockey, and working out, supporting the University of Arkansas Razorbacks in all sports, and spending time with his growing family–a wife, a two-year-old son, and a daughter on the way.
Amy Heinrich
Amy Heinrich, PTA
Amy Heinrich is a licensed Physical Therapist Assistant who received her BS in exercise science from Sam Houston State University in 2014 and continued her education at Houston Community College where she continued her passion for physical therapy and earned her degree.
Outside of the clinical setting, Amy enjoys outdoor adventures such as hiking and cycling around Houston. She loves playing sports and training for triathlons.
Tan Nguyen, CPT, PTA

Tan brings a wealth of experience in personal training and physical therapy to develop unique programs that help his patients surpass their expectations and goals. Tan’s passion is simple: to help patients achieve maximum potential while living a pain-free life. Tan continues to research the newest treatment methods, to ensure his patients are treated with a proven and up-to-date approach.

Tan enjoys spending his downtime with his family, working out, snowboarding and oftentimes you can find him teaching physical therapy students.

Jaylon Smith, PTA

Jaylon started off as a boxing instructor for both, personal training, and group classes at Title Boxing Club. Through this experience, he had a desire to learn more about the body, movement, and rehab in order to further his ability to serve others. He then graduated from Houston Community College as a Physical Therapist Assistant in May 2022 and started with Revolve Physical Therapy May 2022. Jaylon is a servant before anything else. His mission is to utilize, develop, and sharpen whatever skills he has acquired to better anyone he encounters.

In his down time, Jaylon enjoys lifting weights, boxing, running, going to church, and spending time with family/loved ones.

Jose Martinez, PTA

Jose graduated from the University of Houston with a Bachelor’s Degree in Biology. He then attended the Physical Therapist Assistant Program at Houston Community College.

Outside of work, Jose enjoys learning about different styles of martial arts. He also loves spending time with friends and family.

Kunal Shah PTA, CPT, CSCS

Kunal has always been motivated to assist and educate as much as possible. As a lifelong participant (and fan) of all sports, Kunal knows the wear and tear a body potentially goes through and what it takes to help expedite the recovery process whether it be for a week- end warrior with a hamstring strain or a retiree with a hurt back.

On his downtime, Kunal enjoys playing sports and spending quality time with his family and friends.
